Authorization: I hereby grant OTM Performance LLC permission to take photographs and videos of me/my child during training sessions, events, or other activities associated with OTM Performance LLC. I also consent to the recording of my/my child’s comments and testimonials.

Usage: I authorize OTM Performance LLC to use, reproduce, and publish these photographs, videos, and comments in various media, including but not limited to social media, websites, promotional materials, advertisements, and newsletters. This usage is granted without any compensation to me.

Extent of Use: I understand that these photographs, videos, and comments may be used in whole or in part, and may be edited or modified for quality and suitability. OTM Performance LLC will ensure that the content is used in a respectful and appropriate manner.

Confidentiality: I understand that OTM Performance LLC will not disclose any personal information (such as full names or contact details) alongside the media without further explicit consent.

Duration: This consent is granted indefinitely unless I revoke it in writing. I understand that I can withdraw my consent at any time by providing written notice to OTM Performance LLC, and OTM Performance LLC will cease further use of the media featuring me/my child upon receipt of the notice.

Waiver and Release: I release and discharge OTM Performance LLC, its owners, employees, and agents from any and all claims, demands, or causes of action that I may have by reason of this authorization.
