I, the undersigned, hereby acknowledge that I have voluntarily chosen to participate in a program of progressive physical exercise at OTM Performance LLC. I understand that such exercise programs can involve physical exertion and that my participation carries inherent risks, including but not limited to muscle strains, sprains, and cardiovascular events.
I expressly agree and promise to accept and assume all risks associated with my participation in the training program. I hereby voluntarily release, discharge, and hold harmless OTM Performance LLC, its trainers, employees, and agents from any and all claims, demands, or causes of action arising from my participation in the training program.
